Internal liaison

In some cases, elderly patients end up in other hospital departments , e.g. cardiology for significant heart problems. These over-75s are tracked and, if necessary, followed up by an interdisciplinary geriatric team of internal liaisons.

This team consists of:

  • a geriatric specialist,
  • a geriatric nurse,
  • a speech therapist,
  • an occupational therapist,
  • a dietitian
  • a psychologist.

Those who can rely on the assistance of internal liaison team include the attending physician, the entire ward team, the GP and home care providers.

The Internal Liaison Team’s objective is to optimise the quality of care for elderly patients admitted to a non-geriatric ward. Specifically, they identify the needs and risks of the patient in the non-geriatric ward and provide advice accordingly where needed. These patients can be visited on the ward by our geriatric specialists upon request.
