Certificates

This only applies to Maria Middelares General Hospital.

In the context of the Quality Policy of our hospital each new employee must submit the required relevant certificates before the start of their employment.

For the specialist in training and general practitioner in training the following apply:

1. Master’s degree in medicine
2. Belgian Medical Council registration certificate
3. RIZIV registration (i.e. RIZIV e-mail with the registration of the temporary RIZIV number)
4. ALS-/BLS certificate (optional/if available)

Thank you for providing a copy or scan of these documents to the Medical Board Secretariat.

Dit is enkel van toepassing voor AZ Maria Middelares.

In het kader van het kwaliteitsbeleid van ons ziekenhuis dient elke nieuwe medewerker vóór indiensttreding de nodige relevante getuigschriften aan te leveren.

Voor de arts-specialist in opleiding en huisarts in opleiding zijn dit :

1. Diploma master geneeskunde
2. Inschrijvingsbewijs Orde der Artsen
3. RIZIV-erkenning (= RIZIV-e-mail met toekenning van het tijdelijke RIZIV-nummer)
4. ALS-/BLS-attest (optioneel/indien beschikbaar)

Bedankt om van deze documenten een kopie of scan te bezorgen aan het secretariaat van de medische directie.
