Electronic referral via eForm

The eForm 'Referral Letter Maria Middelares' in the GP package allows GPs to digitally refer patients to Maria Middelares General Hospital. This saves time and paper, but it also ensures better continuity of care. Maria Middelares General Hospital collaborated with the other Ghent hospitals and GP Association Ghent for this purpose.

An eForm is an electronic letter template compiled according to the hospital's needs and expectations. The form largely completes itself, automatically selecting data in the patient's medical profile from the GP's patient record. As a GP, all you need to add is the reason for the referral and the specific question.

This digital form, which is sent securely, complies with the privacy regulations. The eForm arrives as a PDF in the hospital's eHealth box. The department in question will receive a notification. If the referred patient is already registered with the hospital, the letter will automatically end up in his hospital record. This means patients no longer have to bring a referral letter with them to the hospital. Still, it is a good idea to let your patient know that you have sent an electronic referral letter, to which hospital and to which department. To that end, you can print the completed eForm and give it to the patient as a copy.

What is that?

You can receive reports and notifications of admission and discharge in electronic form. The hospital uses both Hector and Mediring and Medibridge/Medimail for this purpose. As a GP, you only need to subscribe to one of these systems.

What data?

  • Laboratory reports, X-rays, APD and Nuclear Medicine
  • Letters prepared by all departments of Maria Middelares General Hospital and St Vincent General Hospital in Deinze
  • Notifications of admission and discharge: please specify explicitly if you wish this to be included

Multidisciplinary Oncology Consultation (MOC)

A Multidisciplinary Oncology Consultation (MOC) is a meeting between different physicians and care providers to design optimum care plans for each cancer patient. Specialists from the disciplines involved examine all tests and examination results, and then they discuss the future treatment plan. The Integrated Cancer Centre of Ghent (ICG) of Maria Middelares General Hospital collaborates with specialists in the E17 Hospital Network for the MOC. In addition to oncologists and physicians from the disciplines involved, GPs, oncology coaches, oncology psychologists and oncology dietitians are welcome to join the meetings. Interested GPs can join the meetings in person or take part by video conference.
